How to Register at SMP

We will normally accept patients from the northern-most side of Wigan, Worthington, Standish, Shevington (no further than the Shevington High School)  and  up to Coppull Moor (no further than the Olive Garden).

If you live within our practice boundaries you will need to fill in a New Patient Registration Form so that your medical notes can be transferred from your previous practice. This form can be completed online and emailed to gp-p92014@nhs.net with a photocopy of some form of proof of address (e.g. a utility bill) and a single photo ID (passport / driving licence).

Alternatively you can print off the paper copy and bring this to the surgery with your ID documents.

There can be delays in the notes getting to us depending on what systems your previous practice used and so if you have quiet a complex medical history, or take a lot of medication, it may be helpful to arrange a New Patient Check with our Health Care Assistant. Please bring a list of all your medication to the appointment and any important details regarding your medical history.

Medical treatment is available from the date of registration.

 

Temporary registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
