Updating Your Details – Standish Medical Practice

If you are already registered with us but have changed your name, address or telephone number, please let us know by emailing us at gp-p92014@nhs.net or write to us. Please enclose a copy of proof of new address or name change if this is relevant (e.g.utility bill, marriage certificate). Alternatively you can complete the online form below. If you move outside the practice area you may need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.
